You might need help at home while also managing mental health, trauma, medications, appointments or other responsibilities. When each part is looked at separately, things can become harder than they need to be.
At Hope & Beyond Care Services, we look at how everything connects. We take time to understand what is happening in your life, work alongside the other people involved and make sure your choices and goals remain part of the picture.
Clinical experience matters when it changes what happens in everyday life.
It can mean noticing a shift in behaviour sooner, understanding what may be behind a difficult day, recognising when extra input is needed or helping different people involved in your care stay on the same page.
We combine mental health nursing, trauma-informed practice and years of disability experience with a practical understanding of daily life. So while your care feels personal, there is informed thinking behind it too.
Hope & Beyond Care Services brings together clinical leadership and years of frontline disability experience. Tracy and Jackson have different professional backgrounds, but share the same focus: understanding what each person needs and building a team that can respond well.
Tracy Zachariya brings over two decades of experience, including over a decade of specialised in mental health nursing, to her role at Hope and Beyond Care Services as Qualified Nurse Psychotherapist. Her career reflects a rare combination of deep clinical expertise, trauma-informed therapeutic practice, and proven leadership — making her a trusted clinical anchor for participants navigating complex mental health, psychosocial, and forensic support needs.
As a qualified EMDR (Eye Movement Desensitisation and Reprocessing) therapist and trauma counsellor, Tracy works at the intersection of clinical nursing and therapeutic care, ensuring participants receive support that is not only clinically sound but genuinely healing.
Jackson Faber is the Managing Director of Hope and Beyond Care Services, bringing over a decade of experience in the disability sector as both a Support Worker and Manager. His unique background — spanning a Bachelor of Commerce, a Bachelor of Software Engineering, and sector qualifications including Certificate IV in Disability and Certificate IV in Mental Health — gives him a rare ability to lead with both genuine care expertise and strong operational and business acumen.
Having worked on the front line before stepping into leadership, Jackson understands the realities of support work first-hand. He now channels that experience into building a well-run, sustainable organisation — combining person-centred values with efficient systems, sound governance, and a clear vision for growth.
As Managing Director, Jackson leads Hope and Beyond Care Services with a commitment to quality care, strong operations, and hope in every step.
We treat every person as an individual, listen to what matters to them and respect their choices, routines, culture and personal boundaries.
We involve participants in decisions about their support and help create more opportunities for independence, confidence and control in everyday life.
We communicate openly, follow through on what we say and take responsibility when something needs attention or improvement.
We combine practical support with strong clinical awareness, clear processes and experienced staff so care is thoughtful, reliable and responsive.
We welcome different backgrounds, experiences and ways of communicating, while making sure everyone in our team understands their responsibilities and follows through on them.
Plans, professional recommendations and routines may all be part of your care, but your preferences matter too. We explain what is happening, listen to what works for you and involve you in decisions about your day-to-day life. Because receiving help should never mean losing your voice.
You shouldn’t have to keep chasing answers or wondering whether something important has been passed on. We value clear communication, accountability and follow-through. If something changes, needs attention or should be shared with someone involved in your care, we make sure it doesn’t simply get lost along the way.
